About Us

Who We Are

Real-life strategy to reach your health goals.

Consortium Health and Rehab Center was started in 2021. We are a neighborhood family practice and Psychiatric Mental Health clinic that provides care throughout the life span. We are dedicated to the physical and mental well-being of our community.

This clinic is owned and operated by a certified registered nurse practitioner (CRNP) (PMHNP-BC) with a doctorate in nursing practice (DNP), specializing in psychiatric mental health (PMHNP-BC), and a family nurse practitioner certification (FNP-BC) who has been in nursing for 26 years. After a personal experience with the disparity in the provision of quality health care to the underserved, the stigma attached to mental health, and the negative effects of untreated mental health conditions in society, Vera decided to open up a clinic to cater to the underserved population for a healthier society. Vera is a very compassionate healthcare provider who is very dedicated to her patients and strives to keep them healthy and the community safe. She always puts her patients first, thus the birth of You First Health and Wellness Services.
